Noise would only occur at 30mph and above.

Ended up being the wheel bearing. Replaced it - noise gone.

The car has 126,000 mi. on it - I know the pads have been replaced a few times.

I had same situation. Could not get a definitive diagnosis but everything I read pointed to a wheel bearing going bad.

Replaced the rt rear wheel bearing and problem resolved.
