Axle Bolts

Has anyone had problems with axle bolts coming loose and axle coming out of transmission??
I replaced a bad engine and after driving around for roughly 100 miles the drivers side axle came off the transmission, got it home and checked the right side and found some of the bolts were also loose. I ended up replacing all the bolts and pretty much spot torqued them. I have yet been able to get a torque wrench in there to properly torque them.
Thanks for any info on this problem and a way to not have it happen again would be appreciated.

Be sure all threads are clean & grease free. put a small dab of Blue thread locker on the axle threads before installing bolts & torque then to spec.

You need to pull each bolt, clean it (use something like spray brake cleaner), blow of the bolts with compressed air, then put a small amount of medium strength (blue) Loctite on them, then torque them to spec. The problems will not return.

I've had these flanges on and off numerous times for reasons I won't get into here. Torquing is easy with a good long solid extension bar done from outside the fender.
