It is actually pretty common for those to fail. I know someone has done a write-up, might have been on rennlist. Doesn't take much for those rivnuts to break. Out of 8 986's I have bought I have seen it on at least half of them. Broke one myself with a screwdriver and there is no way my arms can exert that much pressure.
