LTFT too high

2000 2.7 daily driver with 142K. I have on occasion gotten a CEL (1128/30) SO I bought one of the cheapo amazon blutooth obd2 adapters and loaded the torgue app. After watching Schrodingers Box's fuel trims video (HIGHLY RECOMMEND) I ran the test on my long term and short term fuel trims to help diagnose the problem and sure enough it shows bank one and two ltft at about +17-18% - if I'm reading this right.

Car runs great (knock on wood). But I know this condition will eventually hurt the cats and O2 sensors. So based on you guys experiences what would be the first steps you would take to correct this? Check for vacuum leaks/cracks in the oil tube and AOS hoses. Clean MAF and throttle body. Any suggestions welcomed.
