Your long term fuel trims are way out of wack, they should be no more than about +/-10. Mine were up around 27 and replacing the MAF fixed mine.
If you've not already, you might want to try disconnecting the battery for a while so the old LTFT is lost, then taking it out on a decent length trip. The LTFT won't start to adjust until it's warmed up, but may then come down to a normal range.
If not, you've got something else wrong ...
