Okay some stuff I don't know: Has your engine ever been apart?
Not sure how you're able to see all the chain guides to verify they look new. 5 chains, two plastic guides per chain. You might get a look at the ones between the cam shafts but you'd have to be an ant to see much of the others.
First guess? Pressed on sprocket on the IMS has been known to slip a bit (or a lot). I pinned mine with a set screw using a procedure I found on line
