You measure the total thickness, ie both inner and outer (plus the gap between).

My manual (covers '97 - '04) indicates different wear limits, depending on which model:

Boxster

Front: 22 mm
Rear: 18 mm

Boxster S

Front: 26 mm
Rear: 22 mm
