Actually, I only think #1 is a problem. My '01 has done both #2 and #3 ever since I bought it 5 years ago. Changing the fuel filter would be a good place to start. It might not be getting enough fuel due to the filter being clogged. My gatorback belt was super tight when I put it on too. It stretches a bit after awhile.
