Oddly I've never had this problem, and I'm 5'11". Could you maybe raise the seat a hair? My seat is definitely all the way down, but I think I just have a tall torso and short legs.

Since you're getting codes on both cylinder banks, I'm thinking it's worth cleaning your MAF first. Definitely a good place to start at least.
