Clutch slipping at 4k on highway in 4th gear???

So this morning I was getting on the expressway and coming down a ramp. I was in 3rd gear and gassed it to accelerate onto the freeway. I go to shift into 4th gear and realize the tach is moving slowly up in revs at 4k but then slowly goes down about 200 revs and then starts to catch and accelerate. I thought maybe I left my foot on the clutch at top of pedal. I then go to shift into 5th gear and the same thing happens. At this point I know it's not me. Traffic slows and I down shift to 4th again and the same thing happens.
Is there any adjustments that could be done or does this sound like the begining stages of clutch replacement?
Ironically enough on the way home from my son's parade, I did a 4th gear pull and no slippage.

You may need to bleed the clutch slave cylinder with fresh fluid. (Brake fluid)

Thanks BY as that is a good cheap replacement. I just pulled some of the records that came with the car and the clutch was replaced about 30k miles ago along with IMS from LN. Oddly enough the RMS was not replaced and it looks like it is leaking. Maybe 1 drop after I get car back into garage from a drive. Think I read somewhere that oil leak could cause slippage as well?

If the RMS leaks so much that oil is getting on the clutch you'll not only need a new RMS seal but also a new clutch.

I don't think your problem are caused by a master or slave cylinder or can be fixed by a clutch fluid bleed. This would cause additional symptoms like a pulsing clutch pedal for example.

clutch is worn out or contaminated with engine oil or transmission oil

Thanks for the info. Drove car today and unable to get it to slip in 3rd or 4th gear at 4k. Will make plans to drop tranny to get after that RMS.

With the tranny in, you don’t know if the leaking is coming from the RMS or the IMS. Sorry...
