Uh oh... Pulled Intake and now have vacuum lines flopping around

Gents,
Daily driver ('00 base) that has had a vacuum leak for some time now.. decided to check it out today while the weather was nice. Well I could definitely hear a loud hissing when idling and top in service position. I made the mistake of pulling the rubber boot thing between the manifolds (the one closest to the driver) and apparently there's two vacuum lines coming off the bottom. There is a little motor looking thing that controls a big flap in the middle of this intake tube. It appears that a vacuum line should connect to this somehow but I don't see a vacuum line anywhere that would connect to it. The other vacuum line coming off the bottom has a 'Y' connector and one end is connected to something already, and the other connects to the 4-way connector on the canister. So I don't know what needs to connect to that motorized flap thing??

Scared because this is my daily driver and I'm losing daylight! Help!

Garage
On a 2003, Driver side intake manifold has a vacuum switch between cylinder 4 and 5 intake risers. A line comes off the bottom of that switch and goes to the Y connector near the vacuum tank the other end goes to a connection on the underside of the passenger side plenum collar. The same switch has a second tube that goes to the diaphragm you pictured. I have an engine apart now and can probably get you a couple pictures

Appreciate that pic, but it's a little different than mine. I did manage to find the "missing" hose that connected to that motor flap and ended up adding in about 3" of extra tubing as it seemed too short and was "kinking" when put back together (I actually thought this might have been the source of the vacuum leak).

Managed to get it all back together before the sun went down and took it for a spin to test my work. Upon initial start, I got all excited because it sounded like the hissing was gone. Idle seemed fine and didn't appear to hunt at all. While driving it though, the idle dips even lower now when decelerating to a stop. Once I stop completely, it dips to about 250RPM, surges a couple times between 250 and 400ish then levels off around 500-600 steady. Goin in reverse, the idle is really low too, around 250-300.

Guess I'll try to get it to the stealership this week and have a proper test done once and for all (i've been chasing this since i bought the car it seems... years).

Just glad it's semi back together for now!

Here's a vid I took prior to the replacement of all the vacuum lines (the original condition)

You can hear the "surging" and hissing just sitting idle.
