I can't seem to be able to ID this AOS, anyone have any ideas? Thanks

You have one of the originals... round disc on left in pic. I can't recall specifically but I think they are on version 4 or 5 at this point. From version 2 forward, the 'disc' was turned vertical.

Good idea to change that one out. :)
